INTERNSHIP ASSESSMENT:

Posted in GENERAL MEDICINE department from 12/12/22 to 11/2/23 .

In OP:

Took brief history of the patients , done systemic examination (CVS, CNS, RESPIRATORY, PER ABDOMEN).

Ordered necessary investigations according to the symptoms.

Learnt to prescribe medications according to the symptoms of the patient.


UNIT DUTY:

Drawn venous blood samples , collected reports of the samples sent.

Got blood culture bottles and swabs from the lab and collected culture reports from microbiology department.

Updated the reports in WhatsApp groups, investigation charts 

Updated the fever charts everyday.

Got the ECG , 2D echo , x rays done for the patients.

Initiated and monitored blood transfusion of a patient.

Monitored vitals of the patients in the ward , ICU , AMC.

Typed and printed discharge summaries of the patients

This is a case of 60 F with altered sensorium secondary to dyselectrolytemia.

Learnt WHEN and HOW to correct HYPONATREMIA and more importantly when to STOP the correction.

Learnt how and when to correct HYPOKALEMIA .

Learnt how to correct HYPOCALCEMIA.

Demonstrated  trousier’s sign on this patient.

38 year old F , long distance patient with recurrent urethral stricture .

Already underwent urethral dilatation but presented again with urinary retention due to wrong technique of using hegars dilators.

I was able to palpate the bladder which was extending till umbilicus.

Foleys catheterisation was done under deep sedation by Urologist (1500 ml urine was retained)

Monitored blood transfusion of this patient.

Planned for urethroplasty

This is a case of 60 F with diaphragmatic palsy with eventration.

Saw a pt with DEXTROCARDIA for the first time.

Learnt how and when to put a patient on CPAP and significance of it.

This is a case of 60 F with DSS.

Demonstrated HESS TEST on this patient.

Understood the importance of fluid replacement in pt with DSS

Learnt when and how to put the the pt on NORAD and when to taper it.


WARD DUTY:

Allotted cases to the juniors for their exams.

Done examination duty.

I have attended rounds around the patient.


ICU DUTY:

Monitored vitals ( BP , PR ,TEMP , STATS, GRBS) of the ICU and AMC patients.

Demonstrated HESS test on a patient.



Done ASCITIC TAP on a patient.

Learnt drawing ABG samples on the patients ( from radial and femoral arteries)

Inserted RYLES tube.

Done FOLEYS catheterisation on male and female patients.

Placed IV CANNULA.

Assisted in CPR.Learnt when and how to use a defibrillator.

Monitored blood transfusion.


NEPHROLOGY DUTY:

Assisted CENTRAL LINE on 2 patients (under DR PAVANI )

Drawn samples and collected reports .

Monitored patients undergoing dialysis 

Monitored vitals of the CKD patients .

Typed and printed discharge summaries of the patients .
